    Acidentes do trabalho em estudantes do ensino fundamental e médio do Estado de São Paulo

    OBJECTIVES: To estimate the prevalence of occupational injuries and identify their risk factors among students in two municipalities. METHODS: A cross-sectional survey was conducted in public schools of the municipalities of Santo Antonio do Pinhal and Monteiro Lobato, Brazil. A stratified probabilistic sample was drawn from public middle and high schools of the study municipalities. A total of 781 students aged 11 to 19 years participated in the study. Students attending middle and high school answered a comprehensive questionnaire on living and working conditions, as well as aspects of work injuries, and health conditions. Multiple logistic regression models were fitted to estimate risk factors of previous and present occupational injuries. RESULTS: Of 781 students, 604 previously had or currently have jobs and 47% reported previous injuries. Among current workers (n=555), 38% reported injuries on their current job. Risk factors for work injuries with statistically significant odds ratio >;2.0 included attending evening school, working as a housekeeper, waiter or brickmaker, and with potentially dangerous machines. CONCLUSIONS: The study results reinforce the need of restricting adolescent work and support communities to implement social promotion programs.OBJETIVOS: Estimar a prevalência de acidentes de trabalho entre estudantes adolescentes em área urbana e identificar os fatores de risco de ocorrência dos acidentes de trabalho. MÉTODOS: Um estudo transversal foi conduzido nas escolas públicas de ensino fundamental e médio, das cidades de Santo Antonio do Pinhal e Monteiro Lobato, Estado de São Paulo. Foi realizada uma amostra probabilística estratificada em duas escolas públicas de ensino médio de cada uma das duas cidades pesquisadas. O total da população pesquisada foi de 781 estudantes, de 11 a 19 anos de idade. Os estudantes responderam um questionário sobre condições de vida e trabalho, incluindo as características dos acidentes de trabalho ocorridos e problemas de saúde. Foram estimados os fatores de riscos para a ocorrência de acidentes no trabalho, do passado e do presente, usando modelos de regressão logística múltipla. RESULTADOS: Dos 781 estudantes que participaram da pesquisa, 604 relataram ter experiência de trabalho. Destes, 47% relataram já ter sofrido acidentes. Entre aqueles que, no período de coleta de dados, estavam trabalhando (n=555), 38% afirmaram ter sofrido acidentes no posto que ocupavam naquele momento. Os fatores de risco para a ocorrência de acidentes de trabalho, que apresentaram razão de chances acima de 2,0, foram: estudar no período vespertino, exercer funções de empregado(a) doméstico(a), garçom ou oleiro, e trabalhar com objetos e máquinas perigosas. CONCLUSÕES: Os resultados mostram a necessidade de restringir o trabalho de adolescentes, dando apoio às comunidades na implementação de programas de desenvolvimento social

    One World, One Health Challenge: The Holistic Understanding of Rickettsiosis Integrating Multi-Criteria Analysis Techniques and Spatial Statistics

    Among the human diseases caused by etiological agents transmitted by ticks in Brazil, the most prevalent and public health interest is that one caused by species of Rickettsia—Spotted Fever (SF). We applied the concept “One World, One Health” to achieve a proper understanding of SF and determine risk scenarios for human infection by pathogenic Rickettsia in the state of Rio de Janeiro (RJ). Multi-criteria decision analysis and spatial statistics were performed on data encompassing epidemiology, health care, biotic determents, and socioeconomic and demographic variables. The construction of multi-criteria descriptors used 33 indicators ordered in 12 sub criteria of 5 major categories: public health, environmental health, acarology, veterinary health, and microbiology. SF happened nonrandomly in RJ and the risk was heterogeneous in the weight of indicators contribution mainly associated with the criteria acarology (35.11%), microbiology (33.25%), and veterinary medicine (23.96%). Spatially, and according to the PROMETHEE algorithm, ticks from Amblyomma sculptum class and dogs are determinants for the occurrence of human cases of SF in RJ. To define indicators of SF and plan health actions, the “One World, One Health” concept proved to be applicable at three levels: (i) the local or possible areas of infection, (ii) the population or municipality, and (iii) the ecosystem or state. The model of study is flexible according to the reality of the endemic areas and also demonstrates its applicability from a national to a local (home) scale

    Quantification and source identification of atmospheric particulate polycyclic aromatic hydrocarbons and their dry deposition fluxes at three sites in Salvador Basin, Brazil, impacted by mobile and stationary sources

    The present work has aimed to determine the 16 US EPA priority PAH atmospheric particulate matter levels present in three sites around Salvador, Bahia: (i) Lapa bus station, strongly impacted by heavy-duty diesel vehicles; (ii) Aratu harbor, impacted by an intense movement of goods, and (iii) Bananeira village on Maré Island, a non vehicle-influenced site with activities such as handcraft work and fisheries. Results indicated that BbF (0.130-6.85 ng m-3) is the PAH with highest concentration in samples from Aratu harbor and Bananeira and CRY (0.075-6.85 ng m-3) presented higher concentrations at Lapa station. PAH sources from studied sites were mainly of anthropogenic origin such as gasoline-fueled light-duty vehicles and diesel-fueled heavy-duty vehicles, discharges in the port, diesel burning from ships, dust ressuspension, indoor soot from cooking, and coal and wood combustion for energy production.O presente trabalho apresenta o estudo sobre 16 HPAs prioritários em material particulado atmosférico de três sítios próximos a Salvador-BA: (i) Estação da Lapa, estação de ônibus da Região Metropolitana de Salvador; (ii) Porto de Aratu, impactado por intenso movimento de matérias-primas e (iii) Bananeira, localizado na Ilha de Maré, vilarejo que tem como principais atividades a pesca e artesanato. Resultados mostraram que o BbF (0,130-6,85 ng m-3) foi o HPA com as concentrações mais elevadas em Aratu e Bananeira e CRY (0,075-6,85 ng m-3) apresentou nível mais elevado na Lapa.     OBJECTIVE: To estimate the annual incidence of non-fatal work injuries according to sociodemographic and occupational variables among housemaids. METHODS: A community-based survey was conducted in a population of 1,650 women aged 10 to 65 years who reported a paid occupation randomly selected in a household sample of the city of Salvador, Brazil. Data was collected through individual questionnaires on living and work conditions and health status. Fisher Exact test was performed for frequency analysis. RESULTS: It was estimated an overall annual incidence of non-fatal work injuries in the study population of 5.0%, which was statistically significant (pOBJETIVO: Estimar a incidência anual de acidentes não fatais de acordo com variáveis sociodemográficas e ocupacionais entre empregadas em serviços domésticos. MÉTODOS: Inquérito de base comunitária conduzido com 1.650 mulheres de 10 a 65 anos de idade, que referiram ter atividade remunerada e que compunham uma amostra aleatória por conglomerados dos domicílios da cidade de Salvador, capital da Bahia. Os dados foram obtidos por meio de questionários individuais sobre condições de vida, trabalho e saúde. Foi utilizado o teste Exato de Fisher para diferenças de freqüências.     GenSeed-HMM: A tool for progressive assembly using profile HMMs as seeds and its application in Alpavirinae viral discovery from metagenomic data

    This work reports the development of GenSeed-HMM, a program that implements seed-driven progressive assembly, an approach to reconstruct specific sequences from unassembled data, starting from short nucleotide or protein seed sequences or profile Hidden Markov Models (HMM). The program can use any one of a number of sequence assemblers. Assembly is performed in multiple steps and relatively few reads are used in each cycle, consequently the program demands low computational resources. As a proof-of-concept and to demonstrate the power of HMM-driven progressive assemblies, GenSeed-HMM was applied to metagenomic datasets in the search for diverse ssDNA bacteriophages from the recently described Alpavirinae subfamily. Profile HMMs were built using Alpavirinae-specific regions from multiple sequence alignments using either the viral protein 1 (VP1) (major capsid protein) or VP4 (genome replication initiation protein). These profile HMMs were used by GenSeed-HMM (running Newbler assembler) as seeds to reconstruct viral genomes from sequencing datasets of human fecal samples. All contigs obtained were annotated and taxonomically classified using similarity searches and phylogenetic analyses. The most specific profile HMM seed enabled the reconstruction of 45 partial or complete Alpavirinae genomic sequences. A comparison with conventional (global) assembly of the same original dataset, using Newbler in a standalone execution, revealed that GenSeed-HMM outperformed global genomic assembly in several metrics employed. This approach is capable of detecting organisms that have not been used in the construction of the profile HMM, which opens up the possibility of diagnosing novel viruses, without previous specific information, constituting a de novo diagnosis. Additional applications include, but are not limited to, the specific assembly of extrachromosomal elements such as plastid and mitochondrial genomes from metagenomic data. Profile HMM seeds can also be used to reconstruct specific protein coding genes for gene diversity studies, and to determine all possible gene variants present in a metagenomic sample. Such surveys could be useful to detect the emergence of drug-resistance variants in sensitive environments such as hospitals and animal production facilities, where antibiotics are regularly used. Finally, GenSeed-HMM can be used as an adjunct for gap closure on assembly finishing projects, by using multiple contig ends as anchored seeds

    Effects of simvastatin/ezetimibe on microparticles, endothelial progenitor cells and platelet aggregation in subjects with coronary heart disease under antiplatelet therapy

    It is not known whether the addition of ezetimibe to statins adds cardiovascular protection beyond the expected changes in lipid levels. Subjects with coronary heart disease were treated with four consecutive 1-week courses of therapy (T) and evaluations. The courses were: T1, 100 mg aspirin alone; T2, 100 mg aspirin and 40 mg simvastatin/10 mg ezetimibe; T3, 40 mg simvastatin/10 mg ezetimibe, and 75 mg clopidogrel (300 mg initial loading dose); T4, 75 mg clopidogrel alone. Platelet aggregation was examined in whole blood. Endothelial microparticles (CD51), platelet microparticles (CD42/CD31), and endothelial progenitor cells (CD34/CD133; CDKDR/CD133, or CD34/KDR) were quantified by flow cytometry. Endothelial function was examined by flow-mediated dilation. Comparisons between therapies revealed differences in lipids (T2 and T3T1 and T4, P=0.001). Decreased platelet aggregation was observed after aspirin (arachidonic acid, T1<T3 and T4, P=0.034) and clopidogrel (adenosine, T3 and T4<T1 and T2, P<0.0001) therapy. Simvastatin/ezetimibe diphosphate did not change platelet aggregation, the amount of circulating endothelial and platelet microparticles, or endothelial progenitor cells. Cardiovascular protection following therapy with simvastatin/ezetimibe seems restricted to lipid changes and improvement of endothelial function not affecting the release of microparticles, mobilization of endothelial progenitor cells or decreased platelet aggregation.Universidade Federal de São Paulo (UNIFESP) Escola Paulista de Medicina Departamento de MedicinaUNIFESP, EPM, Depto. de MedicinaSciEL

    O ácido linoleico conjugado (CLA) é um ácido graxo poli-insaturado natural, encontrado principalmente na gordura do leite e na carne de animais ruminantes. Seus isômeros geométricos e de posição c9t11 e t10c12 vêm despertando interesse, uma vez que o primeiro é considerado um agente anticarcinogênico natural e o segundo, um repartidor de nutrientes efetivo. Pesquisas relacionam o CLA a outros efeitos positivos à saúde como a redução de aterosclerose, prevenção e tratamento do diabetes mellitus, estimulação do sistema imunológico, redução da pressão arterial e da gordura corporal com aumento da massa magra. Apresenta propriedade hipocolesteronica além de atuar como poderoso antioxidante. Assim, este trabalho de revisão tem por objetivo o enfoque na produção de CLA, seus benefícios à saúde, bem como os métodos analíticos de quantificação do mesmo em alimentos

    Motile sperm organelle morphology examination (MSOME): intervariation study of normal sperm and sperm with large nuclear vacuoles

    <p>Abstract</p> <p>Background</p> <p>Although the motile sperm organelle morphology examination (MSOME) was developed only as a selection criterion, its application as a method for classifying sperm morphology may represent an improvement in evaluation of semen quality, with potential clinical repercussions. The present study aimed to evaluate individual variations in the motile sperm organelle morphology examination (MSOME) analysis after a time interval.</p> <p>Methods</p> <p>Two semen samples were obtained from 240 men from an unselected group of couples undergoing infertility investigation and treatment. Mean time interval between the two semen evaluations was 119 +/- 102 days. No clinical or surgical treatment was realized between the two observations. Spermatozoa were analyzed at greater than or equal to 8400× magnification by inverted microscope equipped with DIC/Nomarski differential interference contrast optics. At least 200 motile spermatozoa per semen sample were evaluated and percentages of normal spermatozoa and spermatozoa with large nuclear vacuoles (LNV/one or more vacuoles occupying >50% of the sperm nuclear area) were determined. A spermatozoon was classified as morphologically normal when it exhibited a normal nucleus (smooth, symmetric and oval nucleus, width 3.28 +/- 0.20 μm, length 4.75 +/- 0.20 μm/absence of vacuoles occupying >4% of nuclear area) as well as acrosome, post-acrosomal lamina, neck and tail, besides not presenting cytoplasm around the head. One examiner, blinded to subject identity, performed the entire study.</p> <p>Results</p> <p>Mean percentages of morphologically normal and LNV spermatozoa were identical in the two MSOME analyses (1.6 +/- 2.2% vs. 1.6 +/- 2.1% <it>P </it>= 0.83 and 25.2 +/- 19.2% vs. 26.1 +/- 19.0% <it>P </it>= 0.31, respectively). Regression analysis between the two samples revealed significant positive correlation for morphologically normal and for LNV spermatozoa (r = 0.57 95% CI:0.47-0.65 <it>P </it>< 0.0001 and r = 0.50 95% CI:0.38-0.58 <it>P </it>< 0.0001, respectively).</p> <p>Conclusions</p> <p>The significant positive correlation and absence of differences between two sperm samples evaluated after a time interval with respect to normal morphology and LNV spermatozoa indicated that MSOME seems reliable (at least for these two specific sperm forms) for analyzing semen. The present result supports the future use of MSOME as a routine method for semen analysis.</p

    The effects of male age on sperm analysis by motile sperm organelle morphology examination (MSOME)

    <p>Abstract</p> <p>Background</p> <p>This study aimed to investigate the influence of age on sperm quality, as analysed by motile sperm organelle morphology examination (MSOME).</p> <p>Methods</p> <p>Semen samples were collected from 975 men undergoing evaluation or treatment for infertility. Sperm cells were evaluated at 8400× magnification using an inverted microscope equipped with Nomarski (differential interference contrast) optics. Two forms of spermatozoa were considered: normal spermatozoa and spermatozoa with large nuclear vacuoles (LNV, defined as vacuoles occupying > 50% of the sperm nuclear area). At least 200 spermatozoa per sample were evaluated, and the percentages of normal and LNV spermatozoa were determined. The subjects were divided into three groups according to age: Group I, less than or equal to 35 years; Group II, 36-40 years; and Group III, greater than or equal to 41 years.</p> <p>Results</p> <p>There was no difference in the percentages of normal sperm between the two younger (I and II) groups (<it>P ></it>0.05). The percentage of normal sperm in the older group (III) was significantly lower than that in the younger (I and II) groups (<it>P </it>< 0.05). There was no difference in the percentage of LNV spermatozoa between the younger (I and II) groups (<it>P ></it>0.05). The percentage of LNV spermatozoa was significantly higher in the older group (III) than in the younger (I and II) groups (<it>P </it>< 0.05). Regression analysis demonstrated a significant decrease in the incidence of normal sperm with increasing age (<it>P </it>< 0.05; r = -0.10). However, there was a significant positive correlation between the percentage of spermatozoa with LNV and male age (<it>P </it>< 0.05, r = 0.10).</p> <p>Conclusion</p> <p>The results demonstrated a consistent decline in semen quality, as reflected by morphological evaluation by MSOME, with increased age. Considering the relationship between nuclear vacuoles and DNA damage, these age-related changes predict that increased paternal age should be associated with unsuccessful or abnormal pregnancy as a consequence of fertilisation with damaged spermatozoa. Given that sperm nuclear vacuoles can be evaluated more precisely at high magnification, these results support the routine use of MSOME for ICSI as a criterion for semen analysis.</p
